Program Guidance: Special Administrative Procedures
A. Special Leave
Leave taken without award support is permitted for one year. Such leave requires the prior written approval of the GACA Program Officer and will be granted only in unusual situations. Support from other sources is permissible during the period of leave. Such leave does not reduce the total number of months of program support for which an individual is eligible except as necessary due to program changes and/or availability of funds.
B. Change of Institution
Prior approval from the GACA Program Officer is required for a change of institution. When an awardee plans to change institutions, a request must be made in writing to the GACA Program Officer a minimum of three months prior to the institutional change. If the individual is moving to an institution that meets program requirements, the award may be continued, provided the following:
- the awardee submits a revised award application recognizing the commitment of the new institution and a report on activities to date;
- all conditions of the original award are met at the new institution including the presence of a qualified mentor;
- the period of support requested is no more than the time remaining within the original award period; and
- the revised application is submitted to the GACA Program Officer three (3) months in advance of the requested effective date.
C. Termination of Award
When an awardee plans to terminate the award, the GACA Program Officer must be notified in writing at the earliest possible time (but a minimum of three months in advance) so that appropriate instructions can be given for termination. A final progress report is required when the award is terminated.
D. Bureau of Health Professions Program Evaluation
In carrying out its stewardship of human resource related programs, the Bureau of Health Professions may begin requesting information essential to an assessment of the effectiveness of this program. Accordingly, applicants are hereby notified that they may be contacted upon the completion of this award for periodic updates on various aspects of their employment history, publications, support from research grants or contracts, honors and awards, professional activities and other information helpful in evaluating the impact of the program.
