Overview
  • BPR provides continued operational support under the Health Center Program authorized under Section 330 of the Public Health Service Act:
    • Community Health Centers 330 (e);
    • Migrant Health Centers 330 (g);
    • Health Care for the Homeless 330 (h); and
    • Public Housing Primary Care 330 (i)

  •    Organizations whose project period
    does not expire in FY 2009 (on or after October 31, 2008 but BEFORE
  •    October 1, 2009).

Summary of FY 2009 BPR Guidance Revisions
  • New due dates for Grants.Gov and EHB have been established;


  • All Program Specific Forms must now be completed electronically within the EHBs;


  • Instructions for the Health Care and Business Plans have been revised and are included in Appendix E;


  • Applicants are no longer required to print, sign and scan the
    SF-424 face page from Grants.gov and upload it as Attachment 1.  HRSA can use the “electronic signature” from Grants.gov as the official signature.  The electronic certification will be considered to be just as “binding” as a non-electronic paper signature; and


  • Form 8A and 8B – Combined to one Form 8 (Certification and Checklist)

  • No Longer Required Forms:
    • Form 1, Part B – BPHC Funding Request Summary;
    • Form 4, Community Characteristics;
    • Form 5, Parts A, B and C – These forms are no longer required as this information will be pre-populated following the completion of the grantee’s scope verification;
    • Form 6 – Part B, Request for Waiver of Governance Requirements;
    • Form 7 – Compliance Checklist;
    • Form 10 – Annual Emergency Preparedness and Management Report;
    • Form 11 - FTCA deeming applications

  • Added Attachment 6:  Action Plan Summary (as applicable)
    • If you have participated in a performance review by HRSA’s Office of Performance Review, provide a summary of your Action Plan activities, describe how the activities have been integrated into your grant program and/or operations, and provide a status update on the activities and/or outcomes achieved.

     Electronic Submission
  • Applications must be submitted in Grants.gov and EHB by the established due dates (found on pages 6 and 19 of the guidance); and


  • HRSA will not accept paper applications without prior approval.  Applicants must request an exemption in writing from DGPWaivers@hrsa.gov and provide details as to why they are unable to submit electronically.

  • Plans will be built around a QI model
    • SAC/BPR plans will state current baseline value for the measure
      (if available) and set a quantified goal for the project period


    • 3 year trended data may not be available for new measures


    • Qualitative progress statements recommended


    • Measurement of baselines and/or progress will be automatically entered into future grant submissions once the UDS is in the EHB
