Measures Alignment
  • All Clinical Measures aligned in UDS and SAC/BPR Health Care Plan (HCP)


  • For diabetes measure, the HCP focuses on not poor control (HbA1c < 9), whereas the UDS measure further separates   adequate control (between 7 and 9) from good control (<7)

Implementation of HCP Clinical Measures
  • Establish a baseline for each measure (optional)


  • For prenatal and LBW, use numbers from most recent UDS


  • For the 4 new measures, use current baseline or  a newly created baseline

UDS Sampling Method
  • 1.  Identify patient population to be sampled (N = the universe)


  • 2.  Determine sample size for manual chart review (n = max 70 patients)


  • 3.  Calculate the sample interval
  • Divide the patient population by the sample size to obtain the sample interval.
  • Sample Interval (i)  =  Population size (N)/ Sample size (n)


  • 4.  Conduct systematic sample


  • 5.  Random Start:
  • -   Randomly pick a patient from the first sampling interval.
  • -   Then, select every ith patient (i = sample interval) until the desired sample size is reached.
