Building a Medical Home for Children

The purpose of this module is to provide information and insight regarding the medical home and how it benefits children. It is designed as a guide for users to better understand the medical home and how to build one. Particular focus will be placed on how health information technology (IT) can facilitate the creation and operation of medical homes.
