"Preventing
and Resolving Homelessness Through Successful
Transitions for Youth and Young Adults"

Workgroup Objectives
- Understand the role of mainstream systems in creating, preventing,
and resolving homelessness among youth and young adults
- Identify ingredients of seamless, integrated targeted systems that
address and prevent homelessness among youth and young adults, and identify
what makes States with seamless systems more effective in preventing
and resolving homelessness among youth and young adults
- Identify strategies for increasing access to and availability of age-appropriate
resources for homeless youth
- Expand the pool of existing regional and national resources available
to youth who are at risk of homelessness.
Specifically, workgroup participants focused on the following topics
as part of their work to prevent and resolve homelessness for youth
and young adults: (1) barriers and challenges, (2) strategies to address
barriers and challenges, (3) additional specific populations, (4) ideas
to take back to State homelessness planning efforts, and (5) resources.

(DE), District of Columbia (DC), Florida (FL), Kentucky (KY), Massachusetts
(MA), Minnesota (MN), Missouri (MO), Oregon (OR), Rhode Island (RI),
Tennessee (TN), Vermont (VT), Virgin Islands (VI), and Wisconsin (WI).
State presenters from California and Illinois also participated.
