COLORADO

VISION 1: We envision a homeless service delivery system whereby homeless persons can access the government and private services they need to reach their maximum potential.

VISION 2: We envision coordinated governmental and private systems and policies that allow for full engagement of adequate resources by all public and private providers to create local solutions that ensure all homeless persons have all services needed for each individual to reach his or her maximum potential.

MISSION: To plan, develop, and implement a coordinated and integrated response to chronic homelessness by identifying and eliminating barriers to accessing and receiving culturally competent mainstream services within the present system, to reduce duplication of services, and to unite mainstream public and private homeless service providers across the State in an ongoing effort to break the cycle of homelessness and help homeless persons achieve and maintain maximum self-sufficiency.

GEORGIA

Homeless persons have increased their independence and been restored to the mainstream of society because state and local resources have ensured optimal opportunities through the creation of an integrated and seamless system of quality services.

HAWAII

Our vision is to SOLVE chronic homelessness in Hawaii.

HOW? A comprehensive and integrated system of services for people who are chronically homeless to help them achieve their optimal level of health, safety, and well being.

MAINE

Maine's vision is that each individual in the State of Maine has a readily accessible, comprehensive array of coordinated housing and supportive service options that maximize self-sufficiency and quality of life.

NEW HAMPSHIRE

We have a vision of eliminating/preventing homelessness. In the near term our vision is of a State where partnerships among citizens, local providers, and State policymakers bring about permanent housing in the shortest time possible. We are committed to ensuring access to and utilization of the necessary services at a local level.

NEW YORK

Create a cost-effective integrated continuum of local, State and Federal systems and services that will permit New York State's homeless individuals and families to achieve their highest level of self-sufficiency.

PUERTO RICO

VISION:

  • No more "chronically" homeless citizens by the year 2007 in Puerto Rico.
  • No more "chronically" homeless citizens by the next decade in Puerto Rico.
  • No more "chronically" homeless citizens in Puerto Rico.

MISSION:

To provide a dignified and comprehensive network of public and private agencies that can offer "chronically" homeless persons an opportunity to live life fully by providing accessible, affordable, available, and timely preventative and active specialized and mainstream services and housing.
