Awarded to 20 teams that implemented clinical pharmacy services in the first year of the Collaborative and were able to demonstrate improvements in the delivery of those services.

Health Outcome Management Award
Awarded to 16 teams that reported health outcomes measures for at least 9-10 consecutive months during the first year of the Collaborative and are using the collected information to manage their population of focus.
Life-Saving Patient Safety Award
Awarded to 19 teams that established systems and processes for detecting, identifying and preventing adverse drug events and potential adverse drug events over the course of the first year of the Collaborative.

PSPC Performance Award
Awarded to 13 teams that documented performance and results for their population of focus using data that demonstrated increased clinical pharmacy services, improved health outcomes, and systematic identification and prevention of adverse drug events and potential adverse drug events during the first year of the Collaborative.

El Rio Community Health Center (PSPC Team) received the 2009 American Pharmacists Association Foundation Pinnacle Award, honoring individuals and groups who have made significant contributions to the field of health care quality and medication use.
Sandra Leal, PharmD, (PSPC Faculty) received the 2009 Pharmacist of the Year Award, sponsored by Pfizer, Inc., honoring an Arizona pharmacist who has demonstrated outstanding leadership for the profession of pharmacy and the Arizona Pharmacy Alliance.
Arthur Martinez, MD, (PSPC Faculty) was recognized with a Pharmacy Appreciation Award by the Arizona Pharmacy Alliance, for his interest and concern for the advancement of pharmacy in Arizona.
Richard Hall (PSPC Faculty and Eastern Virginia Medical School Team) was selected as the Virginia Department of Health Star Employee for 2009 for his innovating and forceful leadership in improving the quality and delivery of care to HIV/AIDS patients in the Three Rivers Health District.

The Patient Safety and Clinical Pharmacy Services Collaborative was selected to present at the 2009 American Diabetes Association Annual Disparities Partnership Forum , which explores collaborative methods for decreasing Type II diabetes and obesity in high-risk populations.
