The Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) High Performer Matrix comprises organization types identified as having proven results in:
patient health outcomes,
patient safety, and
clinical pharmacy services.
From a list of more than 90 organizations determined to have leading practices, 34 organizations representing seven geographic regions were selected for site visits, using the following criteria:
Primary criteria
Clinical outcomes data
Data demonstrating effectiveness of their patient safety and pharmacy services
Innovative patient safety and pharmacy services models
Implemented in outpatient primary care settings
Secondary criteria
Regions with clusters of three or four leading practices in each area (as required by project design)
Regional representation
The learning and results at these sites will develop the Change Package, or menu of leading practices, for the PSPC and the teams that enroll. The matrix includes organization types (e.g., community health center, critical access hospital, hospital or university based clinic) and organizational characteristics (e.g., HRSA supported grantee, rural, urban, or 340B participant).
