Network Partners: The Connecting Communities for Better Health (CCBH) Program is currently evaluating and selecting projects for funding through the Program. Each project selected for funding will have network partners to implement community based health information exchange. The Foundation for eHealth Initiative, however, seeks to align closely with and leverage related health information technology activities in the public and private sectors to further the interests of the National Health Information Infrastructure (NHII).

Project Purpose: The primary purpose of CCBH is to advance the NHII through a set of community demonstration projects and related evaluation and dissemination activities to improve the quality, safety and efficiency of healthcare. CCBH not only plans to provide seed funding to a set of communities to implement health information exchange, it also will create a "Community Learning Network" to provide resources and tools for all communities and stakeholders interested in health information exchange.

Outcomes Expected: It is anticipated that demonstration projects funded by the program will show how electronic health information exchange using common health data standards can help patients receive necessary and timely medical treatment and guard against medical errors. Communicating health data electronically also will enable public health officials to more quickly identify and respond to threats from naturally occurring diseases, such as SARS, and potential bioterror attacks. The strategies and experiences of communities receiving funding under the program to implement electronic health information exchange will be evaluated and disseminated broadly with the goal of spurring movement toward an interconnected, electronic NHII.

Service Area: The Program has received 134 applications for funding from 42 states and the District of Columbia. The specific service areas will not be identified until projects are selected for funding through the Program.

Services Provided: The projects selected for funding through the Program will be providing health information exchange and other health care IT services to clinicians and patients.
