Project Purpose: As a group, people with developmental disabilities such as mental retardation are significantly less active and more susceptible to the health and social problems associated with inactivity than the general population. The purpose of this project is to increase the overall level of physical fitness in this group through an exercise program that uses specialized recordkeeping software, motivational software, and interactive computer video support.

Outcomes Expected: 1) Improved Health and Wellness in participants. Standard measures of body mass index, resting heart rate, and blood pressure will be used. 2) Validation Data. Staff will collect daily measures of participant performance in an individualized exercise program that may include aerobic, flexibility, and strength activities (e.g. steps walked, sit and reach in cm, weight lifted, exercise repetitions). Video records of exercise will be kept. 3) Replication Package. Delivery of a replication package that includes: data collection software, protocols for internet-based video monitoring, and protocols for validated behavioral intervention.

Service Area: North Dakota Counties served include: Burleigh, Cass, Morton, and Wells serving 14 MHPSAs.

Services Provided: Physical education, fitness and psychological services.

Equipment: 1) Del internet servers supporting: Wave3 Corporation's Session Audio/Video teleconferencing server software and NDCPD's Online Exercise Support System. 2) Remote site PCs supporting Wave3 Corporation's Session Audio/Video teleconferencing client software, USB cameras, and Echo canceling microphones.

Transmission: Full T1 to Internet, full T1 at three remote sites, and DSL at the fourth site.

Network Partners: Licensed rural community and hospital pharmacies, rural communities, North Dakota State Board of Pharmacy, and North Dakota State Pharmaceutical Association. Project Purpose: To restore and retain pharmacy services in underserved rural communities in North Dakota through the use of telepharmacy technology. To allow a registered pharmacy technician to process prescriptions in a full service pharmacy at a remote telepharmacy spoke site under the direct supervision of a licensed pharmacist located at a central pharmacy hub site. Activities are in full compliance with all rules and regulations for the practice of pharmacy in the State as established by the North Dakota State Board of Pharmacy.

Outcomes Expected: To restore access to health care, pharmacy services, and pharmacists in remote rural communities throughout the State; to improve rural economic development by building new businesses and adding new jobs; to make rural community pharmacy businesses more attractive and financially viable, profitable, and sustainable; to improve the chances of recruiting and retaining pharmacists in rural areas; to provide educational opportunities in telepharmacy for pharmacy students at the University.

Service Area: 18 rural communities in 17 MUA counties geographically located in western, north central, and southeastern regions of the State serving a total population of 20,203. Communities involved include: Killdeer, Beach, New England, Rugby, Maddock, Rolette, Forman, Gwinner, Watford City, New Town, LaMoure, Enderlin, Lisbon, Oakes, New Salem, Mohall, Minot, and Bismarck.

Services Provided: Full service pharmacies including complete inventory of prescription and nonprescription drugs which process prescriptions, provide drug utilization reviews as well as mandatory patient education counseling and other health, beauty aids, and convenience items.

Equipment: Pharmacy operations software on a standard PC computer; digital imaging camera, and Polycom- FX video conferencing equipment located at both remote telepharmacy spoke site and central pharmacy hub site.

Transmission: Transmitted over the Internet using DSL lines at 512K bandwidth or dedicated fractional T1 and secured through a VPN.

Project Purpose: Provide health-related services at provider, patient and community request including clinical visits, clinical consults, professional and community education and administrative functions.

Outcomes Expected: Clinical outcomes for all services would be equal to outcomes expected for outpatient care within the medical center. Speech therapy outcomes are those used by therapists to determine quality of life improvement in and return as much as possible to normalcy. Speech therapists use a specific rehabilitation assessment tool to gauge these. For educational outcomes include students graduating from the paramedic program and/or other educational objectives being met. Outcomes from all activities is 100% customer satisfaction that is measured on evaluation tools used specifically for each audience (providers, patients, customers).

Service Area: Central and Western ND, Eastern Montana and North Central South Dakota.

Services Provided: Specialty consults to rural physicians; specialty visits to rural patients; speech therapy, medication management and wound management to nursing homes; paramedic education to assist in keeping rural ambulance squads manned; professional education to physicians and other staff at clinics, nursing homes and hospitals, including leadership training; videoconferencing services for administrative meetings throughout the network so that members can avoid needless travel. A number of support groups meet system-wide (e.g. Alzheimer's) and care conferences among professionals are conducted at various facilities. In addition, follow-up burn care is provided for the Regions Medical Center in Minneapolis to reduce patient out-of-state travel.

Equipment: NEC TeleDocs with Canon exam cameras, and Kodak digitizers for tele-radiology purposes. Moving to PolyCom for network purposes and a PACS system for tele-radiology.

Transmission: Point to point T1 lines to all sites. Some sites are piggy-backed on these lines. Northland is in the process of moving toward an ATM network using video over IP.
