The My Health Care Visit section is found on pages 9–10 of the Guide. This is on pages 11–12 of the Guide for Young Women.

This is the only section of the Guide that you complete. This should be done during the health care visit.

Specifically, this section includes spaces for you or another staff member to fill in a patient's BMI and other measurements, including height and weight. It also includes spaces for physical activity and healthy eating assessment and recommendations.
