Page 11 in the Guide for Young Women also prompts you or another clinical staff member to measure or calculate the patient's height, weight, and BMI.
The patient's BMI-for-age percentile should also be recorded to identify whether the patient is overweight, at risk of overweight, at a healthy weight, or underweight for her age.
For young women, calculating BMI and determining the age percentile that the patient falls into is a two-step process:

Step 1Locate the BMI formula found on the CDC chart "BMI-for-Age Percentiles" and calculate the patient's BMI. You will need a calculator for this. Alternatively, an online BMI calculator is available at www.shapeup.org/oap/entry.php. Record the BMI in the Guide
