It includes the Web site addresses of physical activity and healthy eating campaigns and materials.

You can also encourage your patient to think about other resources in her neighborhood that she can go to for more information or help with reaching her goals. Neighborhood resources may be the public library, health newsletters or brochures in grocery stores, or the community recreation center.
