My Bright Future - Physical Activity and Healthy Eating for Rural Young Women

Talking With My Health Care Team

Your health care team (e.g., nurse, nurse practitioner, dietician, promotora, physician assistant, health educator, or doctor) can help you get started. He/she can look at the booklet pages you filled out and then complete "My Health Care Visit: Advice," which shows where you are now in your health and how you can maintain or improve your health. To work with your health care team for your bright future, ask for some time to talk about healthy eating and physical activity when you go for your next appointment. Bring this booklet and ask the questions you write in below. Most team members will answer them gladly. You may also follow up by phone if needed.
