Rural Community Programs

The Community-Based Division (CBD) grant programs provide funding to increase access to care in rural communities and to address their unique health care challenges. Most of CBD’s programs require community organizations to share resources and expertise using evidence-based models of care exit disclaimer in networks of two or more health care services providers.
