Phase 4 and ARP Rural Payments

The deadline to initiate an application for both Provider Relief Fund (PRF) Phase 4 and American Rescue Plan (ARP) Rural payments has passed. Applicants who submitted their Tax Identification Number (TIN) for validation prior to the deadline now have until November 3, 2021 to submit a completed application.

Phase 3 Payment Reconsideration

Providers who believe their PRF Phase 3 payment was not calculated correctly according to HRSA's payment methodology will now have an opportunity to request reconsideration.

Reporting Portal

The PRF Reporting Portal is now open for reporting on the use of funds. This portal is for providers who received one or more Provider Relief Fund payments exceeding $10,000, in aggregate, during a Payment Received Period.
