How to Request Provider Relief Fund Records Through FOIA

The Provider Relief Bureau (PRB) proactively posts information regarding Provider Relief Fund (PRF) payments on its website, including a list of all providers that have received and retained PRF payments and their total amount retained across all PRF distributions. The publicly available lists can be found on the PRF Data page and on the CDC's data page regarding the PRF.

In order to receive PRF records related to a specific provider, PRB requests that you provide the following information:
