HRSA awarded over $15 million to 58 awardees.
The Rural Health Care Services Outreach (Outreach) Program grants implement disease prevention and health promotion activities, expand access to care, and improve health outcomes in rural areas. Through collaboration with local networks of partnering organizations, awarded grants will use innovative, evidenced informed models to address the unique needs of the rural communities they serve. The Outreach program consists of two tracks, the Regular Outreach Track and the Healthy Rural Hometown Initiative (HRHI) Track.
Healthy Rural Hometown Initiative Track
HRSA awarded over $5 million through the program’s Healthy Rural Hometown Initiative Track. These awardees work to address the underlying factors contributing to the 5 leading causes of death in the U.S.: heart disease, cancer, unintentional injury, stroke and chronic lower respiratory disease. A focus on maternal health is also included under this track to address rural needs related to maternal health outcomes.
