The Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) and Office of Minority Health (OMH) are partnering through the National Hypertension Control Initiative: Addressing Disparities among Racial and Ethnic Minority Populations (HTN Initiative).1 HRSA plans to offer funding to health centers under this initiative to increase provider and staff engagement in implementing evidence-based practices, including using advanced self-measured blood pressure technology, to increase the number of adult patients with controlled hypertension.

Who can apply

Organizations that are a Health Center Program operational (H80) grant award recipient (health center) may request NHCI-HC funds.
