Who can apply: States, political subdivisions of states, and Indian tribes and tribal organizations (as defined in section 4 of the Indian Self-Determination and Education Assistance Act (25 U.S.C. § 450b)) not currently funded under HRSA-18-122 and HRSA-19-096 are eligible to apply (i.e., existing recipients of HRSA PMHCA awards or other entities within funded states are not eligible to apply). Please see Appendix for link to currently-funded PMHCA projects.

Who can apply: Eligible applicants include any current recipient funded under the HRSA-19-049 Healthy Start Initiative: Eliminating Disparities in Perinatal Health (start date of April 1, 2019).

Who can apply: To be eligible to apply, your organization must be an existing health center receiving Health Center Program operational support (under sections 330(e), (g), (h), and/or (i)) at the time of application and at the time of award.

Who can apply: Current recipients funded under the State Systems Development Initiative Grant Program (HRSA-18-061 and HRSA-18-062) are eligible to apply for supplemental funding under this program.

Who can apply: Entities conducting LEND training programs funded under NOFO HRSA-21-041 (start date of July 1, 2021) are eligible to apply for this competitive supplement.

Who can apply: Eligible applicants include all states and six territories and jurisdictions serving the District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, the Commonwealth of the Northern Mariana Islands, and American Samoa. Nonprofit organizations currently funded in FY 2020 under the MIECHV Program are also eligible to apply if the state for which they were funded to provide MIECHV services in FY 2020 does not apply.

Who can apply: An eligible entity is a community-based ambulatory patient care center that: i. Operates an accredited primary care residency program. Specific examples of eligible outpatient settings include, but are not limited to: • Federally qualified health centers, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)]; • Community mental health centers, as defined in section 1861(ff)(3)(B) of the Social Security Act [42 U.S.C. 1395x(ff)(3)(B)]; • Rural health clinics, as defined in section 1861(aa)(2) of the Social Security Act [42 U.S.C. 1395x(aa)(2)]; • Health centers operated by the Indian Health service, an Indian tribe or tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act [25 U.S.C. 1603]); and • A community-based entity receiving funds under Title X of the PHS Act. The list of entities above is not exhaustive, but does reflect the intent of the program to provide training in settings such as those served by the institutions listed. OR ii. Has collaborated to form a GME consortium that operates an accredited       primary care residency program.

Who can apply: Eligible applicants include all domestic public or private, non-profit or for-profit entities. Eligible entities must be physically located in New York, New Hampshire, Vermont, or Maine, and must have demonstrated experience serving eligible rural counties and rural census tracts in the NBRC service region, as listed in Appendix A. Eligible entities must be located within the state for which they are applying. For example, an applicant organization applying to serve New York must be physically located within New York. Applicants may only apply to serve one of the four states listed above. All planned activities supported by this program must exclusively target the HRSA-designated rural counties and rural census tracts within the NBRC service area, as listed in Appendix A. Within partially rural counties, only HRSA-designated rural census tracts are eligible to receive activities and services supported by this award, as shown in the table above. For your reference, a list of HRSA-designated rural census tracts is available here. Applicants must include in their target rural service area all eligible rural NBRC counties and census tracts for the state in which they are applying, as listed in Appendix A. Note that services and resources supported by this funding must be available and easily accessible throughout the entire target rural service area. However, it is acceptable to more actively focus on communities with disproportionate levels of need within the target rural service area.
