Who can apply: Eligible applicants include any state including the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, the Virgin Islands, American Samoa, and the jurisdictions encompassing the former Trust Territory of the Pacific Islands; or any domestic public or private entity, including an Indian tribe or tribal organization.  Domestic faith-based and community-based organizations are also eligible to apply.

Who can apply: Any domestic public or private entity, including an Indian tribe or tribal organization (as those terms are defined at 25 U.S.C. 450b) is eligible to apply. See 42 CFR § 51a.3(a). Domestic faith-based and community-based organizations are also eligible to apply.

Who can apply: Only domestic public or non-profit institutions of higher learning and public or private non-profit agencies engaged in research or in programs relating to maternal and child health and/or services for children with special health care needs are eligible to apply (42 CFR § 51a.3(b)). Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply. You are required to submit proof of non-profit status as Attachment 5.
