Who can apply: Your organization must be a Health Center Program award recipient4 with an active H80 grant award to apply.

Who can apply: This Congressional Special Initiative is a limited competition program notice. To be eligible for NHHCIA funding, you must be either: • A current NHHCIA award recipient named in Table 1, or, • A new NHHCS applicant certified by POL and serving an island community, or multiple island communities, as specified in Table 1. You must provide proof of certification in Attachment 13 or your application will be considered non-responsive and will not be considered for funding.

Who can apply: You must be a domestic public or nonprofit private entity, as demonstrated through the submission of the Evidence of Non-profit/Public Center Status (Attachment 11), outlined in Section IV.2.vi.7 Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply.

Who can apply: PCHP funding is available to organizations that: • Are receiving Health Center Program operational (H80) grant funding under sections 330(e), (g), (h), and/or (i), • Have at least one permanent operational service delivery site (i.e., sites that operate at a fixed address and are open year round) in one of the targeted geographic locations, and • Did not receive an FY 2020 PCHP award. See the PCHP technical assistance webpage for a list of eligible health centers.

Who can apply: Organizations that are a Health Center Program operational (H80) grant award recipient (health center) may request NHCI-HC funds.
