Who can apply: You can apply if your organization is a: • Public or private entity, including research centers or networks • Community-based organization • Tribal (governments, organizations) You are required to submit proof of non-profit status as Attachment 5 (if applicable). A foreign applicant will need to be affiliated with a U.S. entity (i.e., university, institution) with a U.S. EIN established and recognized by HRSA to be considered a public or nonprofit institution of higher learning or a public or private nonprofit agency.

Who can apply: •Tribes & tribal organizations •Native American tribal governments •Native American tribal organizations •Institutions of higher education - accredited schools of nursing •State-controlled institutions of higher education •Private, nonprofit institutions of higher education •Public entities •State governments, including U.S. territories •County governments •City or township governments •Special district governments •Independent school districts •Nonprofit entities •Nonprofits having a 501(c)(3) IRS status, other than institutions of higher education. •Nonprofits with an IRS status other than 501(c)(3), other than institutions of higher education.

Who can apply: You can apply if your organization is in the United States, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau and is an entity eligible for funding under RWHAP Parts A – F of title XXVI of the Public Health Service (PHS) Act, including a: • Public or private, non-profit • State or local government • Academic institution • Local health department • Non-profit hospital or outpatient clinic • Community health center receiving support under Section 330 of the PHS Act • Community-based organization • Tribal (governments, organizations) with or without federal recognition

Who can apply: You can apply if your organization is in the United States, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau and is a: • public or private, non-profit or for-profit entity • Domestic faith-based or community-based organization • Tribal (governments, organizations) You can be located in an urban area, but your proposed telehealth network must include at least two originating sites located in rural and underserved areas.

Who can apply: Only an academically affiliated physician assistant training program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) may apply. To be eligible, your organization must be in the United States, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau.
