Who can apply: Eligible applicants include any domestic public or private, non-profit entities, including an Indian Tribe or tribal organization (as those terms are defined at 25 U.S.C. § 5304 (formerly cited as 25 U.S.C. § 45b)). See 42 CFR § 51a.3(a). Domestic faith-based and community-based organizations are also eligible to apply. If funded, for-profit organizations are prohibited from earning profit from the federal award (see 45 CFR § 75.216(b)).

Who can apply: Eligible applicants include States and accredited schools of medicine in States. The terms “accredited” and “school of medicine” for the purpose of this funding opportunity (and under 42 U.S.C. 300w-9(c)) have the same meaning as set forth in section 799B(1)(A) and (E) of the Public Health Service Act (42 U.S.C. 295p(1)(A) and (E)). Tribes and tribal organizations are not eligible. Pursuant to section 1910(a) of the PHS Act, no more than three awards under this authority may be made in a state (i.e., to a state or a school of medicine in such state). HRSA will not provide funds to more than three award recipients within each state in a given fiscal year. HRSA will make competitive awards under this announcement in accordance with the rank order established by the objective review committee, but if awarding funds to the next ranking applicant would result in a fourth award to any state, HRSA will skip that applicant and make the award to the next eligible applicant, ensuring compliance with this statutory restriction

Who can apply: Eligible entities are accredited schools of nursing, nursing centers (nurse managed health clinics/centers), academic health centers, state, or local governments and other nonprofit private or public entities determined appropriate by the HHS Secretary, such as Rural Health Clinics, FQHCs, or HRSA-supported health centers. Community-based organizations, tribes, and tribal organizations are also eligible to apply, if otherwise eligible. See Section IV.2.vi.1 for further details. Eligible applicants located in the 50 states of the U.S., the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau, may apply for these funds. Individuals and foreign entities are not eligible for this HRSA award.

Who can apply: Eligibility is limited to public or non-profit institutions of higher learning and public or private non-profit agencies engaged in research or in programs relating to maternal and child health and/or services for children with special health care needs. See 42 CFR  51a.3(b). Faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply. You are required to submit proof of non-profit status as Attachment 5, if applicable. A foreign applicant will need to be affiliated with a U.S. entity (i.e., university, institution)with a U.S. EIN established and recognized by HRSA to be considered a public or nonprofit institution of higher learning or a public or private nonprofit agency.

Who can apply: Eligible applicants include accredited schools of nursing, nursing centers, academic health centers, state, or local governments, and other public or private nonprofit entities determined appropriate by the Secretary, such as HRSA-supported health centers or Rural Health Clinics. Eligible applicants located in the 50 states of the U.S., the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau, may apply for these funds. Domestic community-based organizations, tribes and tribal organizations are also eligible to apply, if otherwise eligible. Under an academic-clinical partnership, either partner can apply under this NOFO, however both partners cannot be award recipients. Individuals and for-profit entities are not eligible applicants under this NOFO.

Who can apply: As identified in section 2652(a)(1) of the PHS Act, the following public and non-profit private entities are eligible to apply: a) Federally-qualified health centers under section 1905(1)(2)(B) of the Social Security Act; b) Grant recipients under section 1001 of the PHS Act (regarding family planning) other than States; c) Comprehensive hemophilia diagnostic and treatment centers; d) Rural health clinics; e) Health facilities operated by or pursuant to a contract with the Indian Health Service; f) Community-based organizations, clinics, hospitals, and other health facilities that provide early intervention services to people who contracted HIV through intravenous drug use; or g) Nonprofit private entities that provide comprehensive primary care services to populations at risk of HIV, including faith-based and community-based organizations.

Who can apply: As identified in section 2652(a)(1) of the PHS Act, the following public and non-profit private entities are eligible to apply: a) Federally-qualified health centers under section 1905(1)(2)(B) of the Social Security Act; b) Grant recipients under section 1001 of the PHS Act (regarding family planning) other than States; c) Comprehensive hemophilia diagnostic and treatment centers; d) Rural health clinics; e) Health facilities operated by or pursuant to a contract with the Indian Health Service; f) Community-based organizations, clinics, hospitals, and other health facilities that provide early intervention services to people who contracted HIV through intravenous drug use; or g) Nonprofit private entities that provide comprehensive primary care services to populations at risk of HIV, including faith-based and community-based organizations.

Who can apply: Eligible applicants must: • Be a Health Center Program award recipient9 with an active H80 grant award. • Propose to operate a new school-based service site in your health center’s current service area or currently operate a school-based service site, as evidenced by: o School Commitment Documentation signed by school or school district officials specifying support for each proposed new or existing school-based service site. o Form 5B: Service Sites indicates “school” site setting for each proposed new or existing school-based service site. • Propose new and/or expanded services at one or more school-based service sites. If you do not already provide mental health services at each proposed school-based service site, you must propose to expand mental health services (in-person or virtual visit) at each proposed school-based service site.

Who can apply: Eligible applicants include domestic public or private, non-profit or for profit entities including domestic faith-based and community-based organizations, tribes and tribal organizations. The applicant organization may be located in a rural or urban area, but must have demonstrated experience serving, or capacity to serve, rural underserved populations included in the Project Abstract section of the application. The applicant organization may not previously have received an award under 42 U.S.C. § 254c(f) (other than a grant for planning activities) for the same or a similar project. However, existing recipients that (1) seek to expand services or expand their service areas, (2) include new or additional network member organizations, or (3) target a new population or new focus area are eligible to apply. Who can apply: Eligible applicants for the ISTP are: • A Teaching Health Center (as defined in PHS Act Section 749A(f)), or • A Federally Qualified Health Center (as defined in section 1905(l)(2)(B) of the Social Security Act)14, or • A Community Mental Health Center (as defined in section 1861(ff)(3)(B) of the Social Security Act), or • A Rural Health Clinic (as defined in section 1861(aa) of the Social Security Act), or • A health center operated by the Indian Health Service, an Indian tribe, a tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act), or • An entity with a demonstrated record of success in providing training for NPs, PAs, health service psychologists, counselors, nurses, and/or social workers (including individuals completing clinical training requirements for licensure) and including entities that serve pediatric populations. For purposes of this NOFO, an entity with a demonstrated record of success in providing training for NPs, PAs, health service psychologists, counselors, nurses, and/or social workers (including individuals completing clinical training requirements for licensure) is an existing accredited training program for one or more of the eligible disciplines that trains practicing professionals. Provide documentation of applicant organization’s eligibility as specified in this section as Attachment 8.
