Who can apply: Eligible entities for the PCTE-CPMH program are an accredited public or nonprofit private hospital, school of medicine or osteopathic medicine, academically affiliated physician assistant training program, or a public or private nonprofit entity which the Secretary has determined is capable of carrying out such a grant. Faith-based and community-based organizations are eligible to apply for these funds if otherwise eligible. Tribes and tribal organizations may apply for these funds, if otherwise eligible.

Who can apply: Any domestic public or private entity, including an Indian tribe or tribal organization (as those terms are defined at 25 U.S.C. § 450b) is eligible to apply. See 42 CFR § 51a.3(a). Domestic faith-based and community-based organizations are also eligible to apply.

Who can apply: Eligible applicants include entities eligible for funding under Parts A - D of Title XXVI of the Public Health Service (PHS) Act, including public and nonprofit private entities; state and local governments; academic institutions; local health departments; nonprofit hospitals and outpatient clinics; and community health centers receiving support under Section 330 of the PHS Act. Domestic faith-based and community-based organizations, tribes, and tribal organizations are also eligible to apply.

Who can apply: You must be a domestic public or nonprofit private entity, as demonstrated through the submission of the Evidence of Non-profit/Public Center Status (Attachment 11), outlined in Section IV.2.vi.7 Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply.

Who can apply: Eligible applicants are accredited schools of nursing with advanced education nursing programs. Any school(s) of nursing affiliated with this application must be accredited at the time of application – and for the duration of the award – by a recognized body or bodies, or a State agency, approved by the Secretary of Education for nurse education accreditation. Applicants must submit official documentation of accreditation in Attachment 1 for all affiliated nursing programs. Eligible applicants must be located in the United States, Guam, Commonwealth of Puerto Rico, Northern Mariana Islands, American Samoa, U.S. Virgin Islands, Federated States of Micronesia, Republic of the Marshall Islands, or the Republic of Palau. Tribes and Tribal organizations may apply for these funds, if otherwise eligible. Individuals and foreign entities are not eligible under this NOFO.

Who can apply: PCHP funding is available to organizations that: • Are receiving Health Center Program operational (H80) grant funding under sections 330(e), (g), (h), and/or (i), • Have at least one permanent operational service delivery site (i.e., sites that operate at a fixed address and are open year round) in one of the targeted geographic locations, and • Did not receive an FY 2020 PCHP award. Who can apply: Eligible applicants include any domestic public or private entity, including research centers or networks. Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply.

Who can apply: States with State Offices of Rural Health are eligible to apply. Only one applicant from each state is eligible, based on the designation of the governor.

Eligible entities may include, but are not limited to, public and private institutions of higher education, public and private health research organizations, foundations, tribes and tribal organizations, and faith-based entities.
