Who can apply: •RWHAP Part A funded Eligible Metropolitan Area (EMA) or Transitional Grant Area (TGA) whose service area includes one or more of the identified 48 HIV high burden counties; and the EMAs of Washington, D.C. and San Juan, PR. •RWHAP Part B funded States/Territories identified as having a substantial rural HIV burden–Alabama, Arkansas, Kentucky, Mississippi, Missouri, Oklahoma, and South Carolina. •The RWHAP Part B Program of the State of Ohio on behalf of Hamilton County. These types of domestic organizations may apply “Domestic” means the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau.

Who can apply: You can apply if your organization: • Provides, or supports the provision of, health care services in rural areas, frontier areas, health professional shortage areas, or medically underserved areas, or to medically underserved populations or Native Americans, including Indian Tribes, Tribal organizations, and urban Indian organizations, and • Is in the United States or is in Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau, and • Is a public or private, non-profit or for-profit entity, a domestic community-based or faith-based organization, or Tribal (governments, organizations including urban Indian organizations).

Who can apply: You can apply if your organization meets all three of the following criteria: 1. Your organization is in the United States and is: • Public or private, non-profit or for-profit 2. Your organization meets the following geographic requirements: • You are an organization physically located in the Delta Region • You are an organization with demonstrated experience serving, or the capacity to serve, rural underserved populations located within the Delta Region; and • Your organization proposes a project serving the rural Delta region. Applicants will be deemed non-responsive and will not be considered for this funding opportunity if the applicant: • Is located outside of the Delta Region; • Submits an application to serve populations outside of HRSA designated rural counties or rural census tracts in the Delta Region, and • Submits more than one application. Who can apply: You can apply if your organization is in the United States and is: • Public or private, non-profit or for-profit • Community-based • Tribal (governments, organizations) 2. You can apply if your organization meets the following geographic requirements: • You are an organization physically located within Maine, New Hampshire, New York, or Vermont (NBRC member states) Appendix B. • You are an organization with demonstrated experience serving, or the capacity to serve, rural underserved populations located in the NBRC Region (see Appendix B). 3. You can apply if your organization meets the following consortium requirements: • You are an organization representing a consortium composed of three or more health care provider organizations, including the applicant organization, with at least 66 percent (two-thirds) of consortium members located in a HRSA-designated rural area, as defined by the Rural Health Grants Eligibility Analyzer. Applicants will be deemed non-responsive and will not be considered for this funding opportunity if the applicant: • is located outside of Maine, New Hampshire, New York, or Vermont; • submits an application to serve populations outside of HRSA designated rural counties or rural census tracts in the NBRC Region (see Appendix B); and • submits more than one application. Who can apply: The following domestic entities are eligible applicants: • Schools of Allopathic Medicine • Schools of Osteopathic Medicine • Schools of Nursing • Schools of Allied Health • Schools of Pharmacy • Schools of Dentistry • Schools of Public Health • Schools of Optometry • Schools of Chiropractic • Schools of Veterinary Medicine • Schools of Podiatric Medicine • Physician Assistant Education Programs The following accredited graduate programs are also eligible applicants: • Health Administration • Behavioral Health and Mental Health Practice including: o Clinical Psychology o Clinical Social Work o Professional Counseling o Marriage and Family Therapy • Additional eligible applicants include: o a health care facility o a program leading to certification as a certified nurse assistant, o a partnership of a school of nursing and health care facility, or o a partnership of a program leading to certification as a certified nurse assistant, and a health care facility. Community-based organizations, Tribes, and Tribal organizations may apply if otherwise eligible. All eligible applicants must be accredited. In Attachment 7 the applicant organization must provide: • a statement that they hold continuing accreditation from the relevant accrediting body and are not under probation, and • the dates of initial accreditation and next accrediting body review. The full letter of accreditation is not required. If a partner institution holds the accreditation for the training program, a letter of agreement should be provided as well.

Who can apply: An eligible entity is a current THCGME Program payment recipient that was awarded funding under HRSA-20-011 that: i. Operates an accredited primary care residency program. Specific examples of eligible outpatient settings include, but are not limited to: • Federally qualified health centers, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)]; • Community mental health centers, as defined in section 1861(ff)(3)(B) of the Social Security Act [42 U.S.C. 1395x(ff)(3)(B)]; • Rural health clinics, as defined in section 1861(aa)(2) of the Social Security Act [42 U.S.C. 1395x(aa)(2)]; • Health centers operated by the Indian Health service, an Indian tribe or tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act [25 U.S.C. 1603]); and • A community-based entity receiving funds under Title X of the PHS Act. The list of entities above is not exhaustive but does reflect the intent of the program to provide training in settings such as those served by the institutions listed.

Who can apply: An eligible entity is a community-based ambulatory patient care center that: i. Will operate an accredited primary care residency program. Specific examples of eligible outpatient settings include, but are not limited to: ? Federally qualified health centers, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)]; ? Community mental health centers, as defined in section 1861(ff)(3)(B) of the Social Security Act [42 U.S.C. 1395x(ff)(3)(B)]; ? Rural health clinics, as defined in section 1861(aa)(2) of the Social Security Act [42 U.S.C. 1395x(aa)(2)]; ? Health centers operated by the Indian Health Service, an Indian tribe or tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act [25 U.S.C. 1603]); and ? An entity receiving funds under Title X of the PHS Act. The list of entities above is not exhaustive, but does reflect the intent of the program to provide training in community-based settings such as those served by the institutions listed.

Who can apply: Eligible applicants include domestic public or private, non-profit or for-profit entities, including faith-based, community-based, tribes and tribal organizations. The applicant organization may be located in a rural or urban area, but must have demonstrated experience serving, or the capacity to serve, rural underserved populations. The applicant organization should describe in detail their experience and/or capacity to serve rural populations in the Project Abstract section of the application. The applicant organization may not previously have received an RHND award (other than a grant for planning activities) for the same or a similar project. However, existing recipients that (1) seek to expand services or expand their service areas, (2) include new or additional network member organizations, or (3) target a new population or new focus area are eligible to apply.

Who can apply: An eligible entity is a community-based ambulatory patient care center that: i. Operates an accredited primary care residency program. Who can apply: 1) You must be a domestic public or private, nonprofit entity, as demonstrated through the submission of the Evidence of Non-profit/Public Center Status (Attachment 11), outlined in Section IV.2.vi.10 Faith-based and community-based organizations, Tribes, and tribal organizations are eligible to apply.11 2) You must propose in the RESPONSE section of the Project Narrative to operate a health center that makes all required primary health care services (see footnote 6) available and accessible in the service area, either directly or through established arrangements, without regard for ability to pay. You may not propose to provide ONLY a single service or any subset of the required primary health care services. 3) You must propose on Form 5A: Services Provided to make General Primary Medical Care available directly (Column I) and/or through formal written contractual agreements in which the health center pays for the service (Column II). 4) You must provide continuity of services, ensuring availability and accessibility of services to residents of the service area, by proposing to serve an announced service area, as well as: a) Patients: The total number of unduplicated patients that you project to serve in calendar year 2024 (January 1 – December 31, 2024) as entered on Form 1A: General Information Worksheet must be at least 75 percent of the SAAT Patient Target. b) Services: You must project patients on Form 1A: General Information Worksheet for each Service Type (e.g., Medical, Mental Health, Enabling) listed for the service area in the SAAT. c) Service Area: If you are a new or competing supplement applicant, you must enter Service Area Zip Codes on Form 5B: Service Sites for service delivery sites (administrative-only sites will not be considered) that:12 • Include a combination of SAAT Service Area Zip Codes where zip code patient percentages total at least 75 percent of the current patients served; or  • Include all SAAT Service Area Zip Codes for the proposed service area, if the sum of all zip code patient percentages is less than 75 percent of the current patients served. d) Populations: You must propose to serve all population types listed in the SAAT (i.e., CHC, MHC, HCH, and/or PHPC) and maintain the funding distribution from the SAAT in the federal funding request on the SF-424A.
