Who can apply: You can apply if your organization is located in a rural county or parish in the Delta region and is: • Public or private, non-profit or for-profit • Community-based • Tribal (governments, organizations) The Delta region includes 252 counties and parishes located across eight states - Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee. To determine if a county or parish in the Delta region is rural visit https://www.ruralhealthinfo.org/am-i-rural. Include proof of your organization’s location in a rural county or parish in the Delta region in Attachment 3.

Who can apply: Eligible applicants are state recipients funded under HRSA-24-002, Medicare Rural Hospital Flexibility Program cooperative agreement. This is a competitive supplemental funding opportunity. Only states eligible for cooperative agreements awarded under HRSA-24-002 are eligible to apply for funding under this notice.

Who can apply: States are eligible to apply for funding under this notice, including states that are current Medicare Rural Hospital Flexibility Program award recipients under HRSA-19-024 or states submitting a new application from a Governor designated entity. HRSA will accept only one application from each state. The Governor designates the organization that may submit an application on behalf of each state.

Who can apply: You can apply if your organization is in the United States, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau and is an entity eligible for funding under RWHAP Parts A – F of title XXVI of the Public Health Service (PHS) Act, including a: • Public or private, non-profit • State or local government • Academic institution • Local health department • Non-profit hospital or outpatient clinic • Community health center receiving support under Section 330 of the PHS Act • Community-based organization • Tribal (governments, organizations) with or without federal recognition

Who can apply: You can apply if your organization is in the United States, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau and is an entity eligible for funding under RWHAP Parts A through F of title XXVI of the Public Health Service (PHS) Act, including: • Public or private, non-profit • State or local government • Academic institution • Local health department • Non-profit hospital or outpatient clinic • Community health center receiving support under Section 330 of the PHS Act • Community-based • Tribal (governments, organizations) with or without federal recognition.

Who can apply: Eligible entities are domestic public or private nonprofit, or for-profit entities. These organizations may include, but are not limited to: 1) rural hospitals; 2) rural community-based ambulatory patient care centers, including Rural Health Clinics; 3) Native American tribal governments (federally recognized), tribal organizations (other than federally recognized tribal governments), or health centers operated by a tribal government or tribal organization; 4) graduate medical education consortiums, including institutions of higher education, such as, schools of allopathic medicine or osteopathic medicine or Historically Black Colleges and Universities (HBCUs).

Who can apply: You can apply if your organization is in the United States, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau, and represents one or more of the following entity types: • Public or private, non-profit or for-profit • Community-based, • Tribal governments (governments, organizations) • The applicant organization may be located in a rural or urban area, but must have demonstrated experience serving, or the capacity to serve, rural underserved populations, as communicated in the applicant organization’s Project Abstract section of the application, describing in detail the applicant organization’s experience and/or capacity to serve rural populations.

Who can apply: Eligible applicants include domestic public, nonprofit, or private organizations, such as: domestic faith-based and community organizations; state governments and universities; colleges; research institutions; hospitals; local governments; and federally recognized tribal governments, tribes, and tribal organizations. • Native American tribal governments (federally recognized) are eligible. • Native American tribal organizations (other than federally recognized tribal governments) are eligible.

Who can apply: Eligible applicants are non-profit or for-profit entities providing prenatal care, labor care, birthing, and postpartum care services in rural areas, frontier areas, or medically underserved areas, or to medically underserved populations or Indian Tribes or Tribal organizations.

Who can apply: Eligible entities include accredited public or nonprofit private hospitals, accredited schools of allopathic medicine or osteopathic medicine, accredited academically affiliated physician assistant training programs, and a residency training program in family medicine, general internal medicine, general pediatrics or combined general internal medicine and general pediatrics (“med-peds”). Tribes and tribal organizations are also eligible to apply if otherwise eligible. Eligible applicants must provide documentation of accreditation from the relevant accrediting body.
