Children’s Hospitals Graduate Medical Education (CHGME) Payment Program

Eligible applicants include Medicare-certified Rural Health Clinics and organizations that own and operate Medicare-certified Rural Health Clinics. Organizations that own and operate multiple RHCs may apply for up to $50,000 per RHC (e.g., an organization that owns and operates three RHCs can apply for a total of up to $150,000 on one application). Award amounts may be higher or lower than $50,000 per RHC depending on the number of RHCs that apply for the RHCVC Program. Applicant organizations with multiple RHC sites may choose to distribute funds equally to RHC sites or pool funds to achieve program goals. HRSA will confirm applicant Employer Identification Number (EIN) and Centers for Medicare and Medicaid Services Certification Number (CCN) to verify eligibility.

HRSA will use the number of verified CCNs included in your application to calculate the number of RHCs and the total award amount for your application. A CCN may only be included on one application. If HRSA receives multiple applications that include the same CCN, HRSA may disqualify one or more applications as duplicates. Eligible applicants include public, nonprofit, or for-profit Medicare-certified RHCs or organizations that own and operate public, nonprofit, or for-profit Medicare-certified RHCs.

Rural Residency Planning and Development Technical Assistance Program

Telehealth Centers of Excellence

The eligible applicants for this funding opportunity are public academic medical centers located in states with high chronic disease prevalence, high poverty rates, and a large percentage of medically underserved areas. For the purposes of this funding notice, limited competition eligible applicants are currently participating in the Telehealth Center of Excellence program and are located in a state where the chronic disease prevalence among Medicare beneficiaries is equal to or above the national prevalence for at least three of the following chronic conditions (cms.gov portal): • High Blood Pressure/Hypertension • Heart Disease • Cancer • Stroke • Diabetes • Chronic Kidney Disease The data file can be found on the Centers for Medicare and Medicaid website, Medicare Chronic Conditions Dashboard. High Poverty Rates – Applicants are located in a state with a Federal Medical Assistance Percentage (FMAP) at or above 65.0 percent. The 2021 FMAP percentages can be found at: FAS FMAP. Medically Underserved Areas – Applicants are located in states where at least 85 percent of the counties (either the entire county or a smaller division within the county) have been designated as a medically underserved area (MUA). Rurality – Applicants may be located in an urban area but must provide telehealth services to rural areas in applicant’s state in order to receive funding. Applicants must include a list of rural health care facilities in state where telehealth services are currently provided. To determine whether the facility is located in a rural area see: Rural Health Grants Eligibility Analyzer. The applicant organization may be located in a rural or urban area, but must have demonstrated experience serving, or the capacity to serve, rural underserved populations. Applicants should list the locations that will be served and identify rural. Please include the rural census tract(s) in the Project Abstract. The applicant organization should also describe their experience and/or capacity serving rural populations in the Project Abstract section of the application. It is important that applicants list the rural locations (or rural census tract(s) if the county is partially rural) that will be served through their proposed projects, as this will be one of the factors

Currently Eligible Hospitals: Includes freestanding children’s hospitals that meet the original eligibility requirements for CHGME payments that were established prior to 5 the Children’s Hospital GME Support Reauthorization Act of 2013. These freestanding children’s hospitals must meet the following criteria: 1) Have a Medicare payment agreement; 2) Are excluded from the Medicare Inpatient Prospective Payment System (IPPS) pursuant to section 1886(d)(1)(B)(iii)5 of the Social Security Act (SSA) and its accompanying regulations; and 3) Participate in an approved GME residency training program as defined in section 1886(h)(5)(A) of the SSA. b. Newly Qualified Hospitals: As per the Children’s Hospital GME Support Reauthorization Act of 2013, a freestanding hospital may be eligible for CHGME payments depending on the level of funding appropriated to the program if it meets the following criteria: 1) Has a Medicare payment agreement; 2) Is excluded from Medicare IPPS pursuant to section 1886(d)(1)(B) of the SSA and its accompanying regulations; 3) Its inpatients are predominantly individuals under 18 years of age; 4) Has an approved medical residency training program as defined in section 1886(h)(5)(A) of the SSA; and 5) Is not otherwise qualified to receive payments as a Currently Eligible Hospital as defined under this section or section 1886(h) of the SSA. For those freestanding children’s hospitals that met the above requirements for Newly Qualified Hospitals as of April 7, 2014, and the Secretary had not previously determined an average number of FTE residents under section 1886(h)(4) of the SSA, the Secretary may establish such number of FTE residents for the purposes of calculating CHGME Payment Program payments.
