Who can apply: Eligible applicants include domestic public, private, and non-profit organizations, including tribes and tribal organizations, faith-based and community-based organizations, and accredited domestic institutions of higher education including public or private non-profit educational entities, such as four-year colleges and universities, community colleges, technical colleges, vocational schools, Historically Black Colleges and Universities (HBCUs), Hispanic Serving Institutions (HSIs), and Tribal Colleges and Universities (TCUs) located in one of the eight states in the DRA region. Eligible entities must be located in the DRA region. The DRA region includes 252 counties and parishes located across eight states - Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee. Applicants do not need to be located in a rural DRA region county or parish, but training must be provided to the rural counties and parishes listed in the Service Area Requirements section .

Who can apply: Eligible applicants include any current recipient funded under the HRSA-19-049 Healthy Start Initiative: Eliminating Disparities in Perinatal Health (start date of April 1, 2019).

Who can apply: Eligible applicants shall be domestic public or private, non-profit or for-profit, entities, including faith-based and community-based organizations, tribes, and tribal organizations. The applicant organization may be located in an urban or rural area but must have demonstrated experience serving, or the capacity to serve, rural underserved populations. However, all activities supported by this program must exclusively target populations residing in HRSA-designated rural counties or rural census tracts in urban counties. To ascertain whether a particular county or census tract is rural, please refer to https://data.hrsa.gov/tools/rural-health?tab=Address. Applicants should list the rural areas (counties) that will be served. Proposed counties should be fully rural, but if counties are partially rural counties, please include the rural census tract(s) in the Project Abstract. The applicant organization should also describe their experience and/or capacity serving rural populations in the Project Abstract section of the application. It is important that applicants list the rural counties (or rural census tract(s) if the county is partially rural) that will be served through  their proposed project, as this will be one of the factors that will determine the applicant organization’s eligibility to apply for this funding.

Who can apply: Under Sections 2620 and 2689(11) of the PHS Act, all “States” – defined as all 50 States, the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, Guam, American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of Palau, the Federated States of Micronesia, and the Republic of the Marshall Islands – are eligible for RWHAP Part B supplemental funding. However, any state/territory that had an unobligated balance of more than five percent of their FY 2019 formula funds is not eligible to apply for the FY 2021 RWHAP State/Territories Part B Supplemental Grant Program. Based on the criteria listed above, the following entity is not eligible to apply to this NOFO: o Federated States of Micronesia

Who can apply: Eligible applicants include nonprofit private or public organizations with demonstrated experience in implementing public health programs and established relationships with such State, territorial, or local public health departments, particularly in medically underserved areas. This includes Tribes and Tribal organizations as well. Applicants should have demonstrated experience and expertise in implementing public health programs across broad geographic areas. Applicants will be expected to demonstrate the ability to provide services to areas and populations not being reached by current COVID-19 vaccination and response efforts. Applicants are expected to have, and should be able to clearly describe the partnerships they have formed at both the regional and local level to directly assist individuals in getting the COVID-19 vaccine. These partnerships should include organizations such as community-based organizations and other health and social service organizations that can directly hire community outreach workers from the vulnerable and medically underserved communities they will serve and can reach out to these communities across the country; specifically the areas or populations with low vaccination rates to date. Applicants should use recent, relevant data sources to identify counties and target populations of high social vulnerability (see Section IV.7). Applicants should propose a multi-state or multi-regional approach with strong local partnerships.

Who can apply: Eligible applicants include domestic and foreign public or private, nonprofit entities, including institutions of higher education, and for-profit entities. Faith-based and community-based organizations, Indian Tribes, and tribal organizations are eligible to apply. If you are an applicant from a U.S. domestic organization, HRSA encourages you to form a consortium of partners to include foreign institutions with the relevant expertise, with the long-term goal of strengthening networks, ensuring sustainability, and providing in-country personnel. If you are a foreign applicant, you may include collaboration with U.S. and other foreign institutions with particular expertise in the proposed priority areas as sub-recipient consortium partners. The applicant institution must meet the eligibility requirements and assumes all legal, programmatic, and financial responsibilities under the award. Consortium participants are sub-recipients under the award. Sub-recipients are subject to all programmatic terms and conditions of the award. Depending on the type of engagement and scope of work, consortium agreements may be in the form of memorandums of understanding (MOUs), sub-awards, or contracts.

Who can apply: Eligible applicants for the BHWD TAE are: Health professions schools, academic health centers, State or local governments, or other public or private nonprofit entities that provide services and training to health professions. A nationally recognized accrediting body, as specified by the U.S. Department of Education, must accredit applicants that are institutions of higher education. These applicants must submit their accreditation documentation as Attachment 4. Accredited health professions schools and academic health centers applicants are required to submit accreditation documentation. Those that fail to attach a copy of the required accreditation documentation as Attachment 4 will be considered non-responsive and will not be considered for funding under this announcement. Applicants are required to maintain their accreditation throughout the project period and notify HRSA of any change in status. Eligible entities must be located in the 50 states, the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, America Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau. Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply, if otherwise eligible.

Who can apply:

The eligible applicants for this funding opportunity are public academic medical centers located in states with high chronic disease prevalence, high poverty rates, and a large percentage of medically underserved areas. For the purposes of this funding notice, limited competition eligible applicants are currently participating in the Telehealth Center of Excellence program and are located in a state where the chronic disease prevalence among Medicare beneficiaries is equal to or above the national prevalence for at least three of the following chronic conditions (cms.gov portal): • High Blood Pressure/Hypertension • Heart Disease • Cancer • Stroke • Diabetes • Chronic Kidney Disease The data file can be found on the Centers for Medicare and Medicaid website, Medicare Chronic Conditions Dashboard. High Poverty Rates – Applicants are located in a state with a Federal Medical Assistance Percentage (FMAP) at or above 65.0 percent. The 2021 FMAP percentages can be found at: FAS FMAP. Medically Underserved Areas – Applicants are located in states where at least 85 percent of the counties (either the entire county or a smaller division within the county) have been designated as a medically underserved area (MUA). Rurality – Applicants may be located in an urban area but must provide telehealth services to rural areas in applicant’s state in order to receive funding. Applicants must include a list of rural health care facilities in state where telehealth services are currently provided. To determine whether the facility is located in a rural area see: Rural Health Grants Eligibility Analyzer. The applicant organization may be located in a rural or urban area, but must have demonstrated experience serving, or the capacity to serve, rural underserved populations. Applicants should list the locations that will be served and identify rural. Please include the rural census tract(s) in the Project Abstract. The applicant organization should also describe their experience and/or capacity serving rural populations in the Project Abstract section of the application. It is important that applicants list the rural locations (or rural census tract(s) if the county is partially rural) that will be served through their proposed projects, as this will be one of the factors

Who can apply: Eligible applicants include all domestic public or private, non-profit or for-profit entities, including faith-based and community-based organizations, and federally-recognized tribes and tribal organizations. In addition to the 50 U.S. states, only organizations in the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, the U.S. Virgin Islands, the Federated State of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau may apply. If you are located outside the 50 states, you must still meet the eligibility requirements. The applicant organization may be located in an urban or rural area and should have the staffing and infrastructure necessary to oversee program activities, serve as the fiscal agent for the award, and ensure that local control for the award is vested in the targeted rural communities. All activities supported by RCORP-PS (i.e., all service delivery sites) must exclusively occur in HRSA-designated rural counties or rural census tracts in urban counties, as defined by the Rural Health Grants Eligibility Analyzer
